Here at Chambers Chiropractic & Acupuncture, we value our patients. We acknowledge that going to a new place for the first time can be nerve racking. We want to do everything we can to eliminate any unnecessary stressors, which is why we include the option of printing and completing your forms at home. First Visit

Our Physicians are dedicated to providing the best service possible for your condition. For that reason we have developed procedures to first determine the cause of your pain. Then we develop a treatment plan that is right for you.

Step 1

Please bring all insurance cards, worker compensation or motor vehicle accident information with you to the first appointment. If you have them, also bring x-rays and MRI’s performed at other offices.

Step 2

Dr. Chambers will perform a thorough consultation and health history in order to determine possible causes of your pain and decide what examination is necessary.

Step 3

A complete examination specific to you condition will be performed to further determine the cause of your pain.

Step 4

The doctor will determine if x-rays or other tests are necessary to diagnose your condition.

Step 5

The doctor will release you with home instructions. You will be given an appointment for the following day to review the findings

Step 6

The next appointment will be a review of findings. You and the doctor will review the exam and x-rays, discuss your condition and treatment options.

Step 7

When a treatment plan is agreed upon, the doctor will have a staff member review your insurance with you. We feel it is your right to understand your benefits.

Step 8

Treatment will begin with this visit and a treatment plan with multiple appointments will be given. It is important that you keep all appointments for the treatment to work.
